⚠ BREAKING CHANGES

the %Exec{} :search field is now a mode atom (:cached, :indexed, :live, :disabled) or nil, where it was a boolean defaulting to false. The emitted argument changes from --search to -c web_search="<mode>". Exec.search/1 is unchanged for callers and now means web_search="live".
Exec.approval_policy/2 no longer accepts :on_failure, which the Codex CLI dropped along with the --ask-for-approval flag. Valid policies are :untrusted, :on_request and :never. The emitted argument changes from --ask-for-approval <policy> to -c approval_policy="<policy>".
CodexWrapper.Commands.Fork is removed. codex fork is an interactive TUI command that cannot be driven non-interactively, so the module never worked against codex-cli 0.14x. Use CodexWrapper.ExecResume to continue a session in place; there is no non-interactive way to branch one.
