Pure Responses codec for text, JSON function and raw custom tools, including
ordered text/image tool outputs. Images use string-keyed data, mime_type
and optional detail fields; see Backplane.AiProtocol.ContentBlock.
Tool result status nil or :success encodes as an ordinary completed output.
Status :error is incompatible: Responses has no semantic is_error field,
and a function item's lifecycle status cannot represent a tool error.
Canonical namespace::name identities use native namespace declarations and call fields.
Unsupported content and unresolved state fail before transport. This codec does not execute
tools or grant authority. Hosts own dispatch and capability selection.
